First Middle and Last Name Confusion for opening a bank account online I am struggling to open up a savings account with Commonwealth Bank of OZ. They are repeatedly asking for my first name, middle name and last name, though I have sent them a copy of my passport too. My problem is my name in full has six parts in it; one family name and five given names.
How can I give my name into this format? Appreciate your comments.
Assume my name in full as: Konara Mudiyanselage Kolitha Yapa Kumara Konara

First Name(s) Konara Mudiyanselag
Surname(s) Kumara Konara
Middle Name(s) Kolitha Yapa
or any other combination you like. But you should try to be consistent when filling forms for other identity documents like Drivers License, Medicare card, Credit card and any other forms you fill. Talking about forms, how did you fill your skills assessment and visa application forms? They asked for the same.

Thanks for your suggestion. Both skills assessment and visa applications had asked only about family name and given names. There I stated my last name as family name and rest of the names as given names.
So my family name was "Konara", and given names were "Konara Mudiyanselage Kolitha Yapa Kumara".
Now when I gave this bank the same format, they are insisting on first name and middle name, for which I am confused. Finally, I proposed last name as "Konara", middle name as "Kumara" and first name as "Konara Mudiyanselage Kolitha Yapa", which I still do not know whether have accepted or not. Kindly propose me the best way to do this to be consistent with visa application's name format.

Re: First Middle and Last Name Confusion for opening a bank account online It is not so important to match with your visa application. The system may have accepted the last set of names you provided. Otherwise it would tell you so then and there while you were filling the onine form.

Re: First Middle and Last Name Confusion for opening a bank account online Now I heard from a friend that VicRoads consider the name appearing on Visa Endorsement page regardless of what the passport holds on bio-data page. My name appears as Konara M. Konara on Visa page apparently due to its length.
I want to have my preferred first name with the family name as my full name, if I have to use a shortened name in OZ. Kindly shed some light on this issue.

Re: First Middle and Last Name Confusion for opening a bank account online Yes it is a common practice to go by first name "middle" initial and last/surname/family name. And I dont see any problem with that. Examples, George W. Bush, John F. Kennedy.
